Scheduled Airlines - This form must be filled in a submitted to Sportif International immediately as we are unable to issue flight tickets without the information contained therein.
Charter Airlines are collecting this information at check-in, please complete this form and take to check-in with you.
It is the responsibility of individual passengers to ensure that they comply with the requirements.
